This form allows us to change your address with HPOPS. Changing your address with the City of Houston does not update your records with HPOPS

Direct Deposit Form
This form allows HPOPS to start, stop, or change the direct deposit of your monthly pension benefit into your bank account. In order to process an electronic deposit HPOPS must have the signed form in our office by the 10th of that month.

Member Information Request Form
This form allows members to request information from HPOPS. Typical requests include Mortgage and Social Security letters, and information used in a divorce.

Tax Withholding Form
This form allows HPOPS to change the tax withholding from your monthly benefit payment. HPOPS cannot assist you in determining the appropriate amount to withhold from your monthly benefit payment.
